ON-LINE TRAINING

The On-Line Training Session consists of the five (5) presentations below.

You may click on any presentation in any order. It may take several hours to complete all five presentations.

Upon completing each presentation you will be asked to print out a certificate of

completion or a question form that you will be required to attach to the cover

sheet (Click Here for link to the Cover Sheet)

and you will need to turn in prior to being admitted to the clinic session.

NO Forms ..... No Admittance !!!!

If for some reason you cannot print out a form please contact Jack Dockery.

This web-site is housed in Ohio so when you hit home on any of the presentations below you will not be taken back here but to Ohio's D module.
